Community Health Nurse - Home Care RN

Position Summary: Community Health Nurse will provide Home Health Care services to the Rochester Regional Home Care clients per Medicare, Medicaid and N.Y.S. Full-Time (40 hours a week) Community Health Nurse

$7,500 Sign-On Bonus for experienced Home Care Nurses and Cardiac Nurses!**

Responsibilities:

  • Maintains a caseload of primary care clients, as assigned, which supports the standard productivity requirements of the program.
  • Provides comprehensive skilled nursing assessments of the bio-psycho-social needs of acutely ill clients in their home.
  • Completes all OASIS assessments per Medicare regulations.
  • Performs skilled nursing care and prescribed treatments to clients based on MD orders.
  • Formulates a comprehensive plan of home care services for assigned clients based on nursing assessment, which addresses medical, environmental, social and psychological needs.
  • Responsible for the provision of ongoing care management to meet clients needs.
  • Coordinates client services and/or referrals in an efficient and timely manner.
  • Monitors and coordinates all client services as it relates to the plan of care.
  • Provides client and family education in accordance with client assessment and prescribed plan of care.
  • Communicates plan of care to the Primary Care Physician (PCP), updates PCP with changes in condition and subsequent alterations to plan of care.
  • Documents and implements physicians orders per D.O.H. and program standards.
  • Establishes paraprofessional care plan and supervises paraprofessional per CMS and DOH regulation.
  • Completes accurately and timely all documentation of assigned clients' care per Federal and State regulatory standards.
  • Participates in CHHA in service education program. Maintains updated knowledge base of community health practice and community resources.
  • Participates in the on-call rotation as outlined in the CHHA Policy and Procedure manual and therein provides quality care management and continuity of care to clients 24 hours seven days per week.

Minimum Qualifications:

  • ASN or BSN and at least one year of nursing experience required, preferably in an acute care setting.
  • BSN and 2 years experience in home care preferred.
  • Bilingual skills preferred.

Required Certification/Licensure:

  • Registered in NYS as a registered nurse professional.
  • Current N.Y.S. Drivers License.

Physical Demands:

  • Continuous standing and/or walking.
  • Frequent bending and twisting of neck and back, reaching below waist level, squatting.
  • Occasional pushing/pulling, lifting portion of patients' weight up to 35 lbs; over 35 lbs requires assistive devices or other employee assistance.
  • Fine manual dexterity for computer use.

Working Conditions:

  • Is required to drive own car in order to meet essential functions as listed.

**Sign-On Bonus is paid out in $1,500 increments every 6 months over 2.5 years.
