Hours of Operation: M-F 8-5, with a flexible weekend rotation schedule

POSITION OVERVIEW

Consumer Direct Care Network Montana- Home Health & Hospice is locally-owned and a national leader in community healthcare and caregiving has an immediate opening for a part-time, full-time and on-call nurses. In your important role, not only will you work closely with the clients and their families, but you will also collaborate with interdisciplinary clinicians and clients’ primary care providers to create a plan of care, implement and evaluate the needs of the patients and their families throughout the duration of service-- providing care that matters the most. WHAT YOU WILL DO


1. Completes and initial assessment of patient and family to determine medical, spiritual and psychosocial needs and regularly re-evaluates client’s needs

2. Initiates the plan of care and makes necessary reviews and client’s status and needs changes

3. Develops individualized plans of care by establishing goals based on nursing diagnosis, therapeutic, preventative and rehabilitative interventions as well as incorporating spiritual and psychosocial needs

4. Administers and/or teaching of medications and treatments as prescribed

5. Provides health care instructions to the clients and/or families as appropriate

6. Identifies discharge planning needs as part of the care plan development and implements prior to discharge patient

7. Counseling and emotional support of patients and families


Communication

1. Prepares clinical notes and updates the primary physician, when necessary

2. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required

3. Communicates on a regular basis with the interdisciplinary team

5. Assists the client and family/ caregiver and other team members in providing continuity of care

QUALIFICATIONS

  • 1yr home health or hospice experience preferred, but willing to train the right candidate. New grads welcome!
  • Current MT RN/LPN license
  • Current CPR certification
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements
  • Must be able to successfully pass a background screening and pre-work screening
